Description:
I have two tables:
• Clients attendance
• Clients

Client&'s Table Structure: ID, Record Number, Last Name, First Name, Team, Counselor, DOB, Sex, and Intake

Client&'s Attendance Table Structure: ID, Date and Record Number.

I need to create a MS ACCESS attendance report showing the following for each client :
• First Name
• Last Name
• Counselor
• Team
• Intake date
• Birthday
• Sex
• Age ( i.e.34 years old)
• Record Number
• First visit (i.e. 5/1/01)
• Last visit (i.e. 11/6/02)
• Total number of visits (i.e. 80)
• Number of visits per week (i.e. 3)
• Number of visits per month (.i.e. 22)
• Number of visits per year (i.e. 55)
• Last 5 consecutive visits (i.e. 5/1 to 5/5).
• Each fifth visit date (i.e. 5/3, 5/8, 5/14, 5/19, 5/24, 5/29. 6/3…).
• Time elapsed between consecutive visits if any (i.e. 6 days, 3 months, etc).
• Time elapsed from last consecutive visit (i.e. 7 days).
• Time elapsed from last visit (i.e. 4 days).
• The largest number of consecutive visits achieved (i.e. 25 consecutive visits).
• Dates of largest amount of consecutive visits achieved (i.e. 5/1/02 to 5/25/02).
The dates above are only examples.


Deliverables:
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.

3) Complete ownership and distribution copyrights to all work purchased.

WINDOWS 98, XP 2000

Must be 100% finished and received by buyer on:
Dec 1, 2002 EDT
Deadline legal notes: All times are expressed in the time zone of the site EDT (UT - 5). If the buyer omitted a time, then the deadline is 11:59:59 PM EDT on the indicated date.

Special Conditions / Other:
Must be complete by 12/1/2002

Hi, this is Scott Tripp, an Access database programmer/consultant for Per-Se Technologies.

I am sure I create report(s) that show this information immediately. The only concern is if all of the information will fit on a report (even in landscape), and even if it does will it convey the information. Specifically, if a single report tires to show all possible information, it may become too cluttered to readily read. In this case, several reports may be a better option.
