Description:
I want a 3D logo/icon for my VB6 client application and website. The application name is TeamOne.It is used to enter and retrieve patients data from a SQL server. When I open it, all I see is a grey window with pulldown menus on the top. It is a very simple application, but it looks dead and ugly. When opening the application, I want to see a profesional and colorful 3D background, computer or healthcare related, with high resolution graphics; with an big 3D high resolution icon on the middle.The icon must use the word "TeamOne". The logo/icon also should be available as a desktop shortcut and printing. The BV6 application is being finished for another RAC coder, so you will be working with him to incorporate the graphics into the application.
I do not have any ideas, so I am leaving it to you to create something. (The application is to be used in a Substance Abuse Acupuncture Clinic).
Deliverables:
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.
3) Complete ownership and distribution copyrights to all work purchased.

Hope you are doing good. I am shahan and I work in a software house called Telelogix(www.telelogix.com)
I m a member of a group of 5 professional graphic and web designers and have also done qualification in graphic desing.
Your project appealed me so much because of 2 resons. Firstly it involves grahics and secondly, it involves VB. I am an expert in Vb and I can very well understand where to put what.
I make make ur VB application lively and colorful but with purpose. I'll infuse a new spirit in ur user interfaces which would definitely make ur application more interactive and attractive.
Now i wont take much of ur time, rather i would let my work speak for me. Plz take a look at my recent work (zip file) to have an idea of my style of work.
