Consultancy PHP Website Using MySQL Database!!

Description:
A recruitment-type of web-based database system for a consultancy company, using a MySQL database. Would have slight "issues", such as the "consultant" CANNOT add, delete or amend their own records due to a ISO-9000 standard, instead the record is flagged (possibly added as a flagged record within the system) and once the details are verified then record is then "live" (unflagged). Client enters on a form, then private details (name, addy and other contact info), then up to 10 countries of experience (i.e. possibly where they have worked etc), enter their "highest" level of qualificaitons (i.e. MSc/MA/MPhil or BSc/Ba and other similar combination options), they then enter their top 3 languages (rating them accordingly for "spearking" and "reading and writing" ability.) Then they select up to 3 major disciplines, and for each one select upto 10 sub-specialities. Database should be fully searchable by administraors or even by third party (but no private info is displayed on a third part search, just "id" number), say for instance someone is looking for a "Change Management" consultant, they search for this and if this is selected as an option for any consultant under any of their "major disclipine" options, their record comes up on the results page. (clear?). Records can only form part of a mail merge (in MS Word), say for all people in a "region" or "specialitiy".
Pls e-mail me if oyu would like a screenshot of the current "access" database model (not normilised!). Looking for a VERY quick time-span for the project, so pls no time-wasters.
P.S. Not sure whether a log-in facility should be provided for "consultants" or not - suggestions? What I initially had in mind was a section where a client can log-in and can add or amend or delete details (or delete the whole record), this request is then submitted into the database AND submitted via e-mail to the company's main e-mail addy. They then click on the URL posted in the msg to confirm or reject the addition, or the amendments (they have told me that somethings the consultants simply 'should not' be added to the database, so a rejection option should be available). Once they have received the necessary 'proof' (validation/verification documentation), they can either accept or reject the record, until then the record is 'flagged' and is not available for searching!
Deliverables:
Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. Complete copyrights to all work purchased.
Explanations of code would also be required and anything else which relates to the design and implementation of the project/code.

Within the next 5 weeks - so quite URGENT!!!
Remember that contacting the other party outside of the site (by email, phone, etc.) on all business projects < $500 (before the buyer's money is escrowed) is a violation of both the software buyer and seller agreements.
We monitor all site activity for such violations and can instantly expel transgressers on the spot, so we thank you in advance for your cooperation.
