Remove default values. 2. The 2nd combo should default to "select" as the 1st one already does. 3. It needs validation code to ensure that all questions have been answered. If one or more haven't been answered then it should give the user a warning message and jump to the first of the missing answers. 4. We also need you to create an access database to hold the form data (and Request.ServerVariables("REMOTE_HOST") and Request.ServerVariables("HTTP_USER_AGENT")). The 2nd form (pref_survey_thanks.htm) needs to be renamed .asp and code added to save the form data to the database. Shouldn't be more than about 3 hours work. However, we need it by Saturday night, so please bid only if you can work on it straight away and know what you are doing! Deliverables: 1) Complete and fully-functional script and database. 2) Complete ownership and distribution copyrights to all work purchased.
