We now need a simplified version of this that allows the user to enter their email address, and the script will send the client the email address entered and the contents of a hidden field "documentname". We already have the form, we just need the script cut down and modified accordingly. I have attached the (fully working) old form and script, and the new form (which needs the hidden field added).
