Description:
We require a database in MS Access Normalised to 5NF. Its for a gym club. The un normalised feilds for the database are as follows.
1/MEMBERSHIP NUMBER, 2/MEMBERSHIP SURNAME, 3/MEMBERSHIP FORENAME, 4/MEMBERSHIP ADDRESS, 5/MEMBERS COURSE, 6/MEMBER HEIGHT, 7/MEMBER WEIGHT, 8/MEMBER E-MAIL ADDRESS, 9/MEMBER TELEPHONE NUMBER. 10/MEMBER SITUP, 11/MEMBER PRESSUP, 12/MEMBER PULSE RATE, 13/MEMBER TIME TO RUN 1 MILE, 14/COURSE, 15/COST OF COURSE, 16/STAFF NUMBER, 17/STAFF SURNAME, 18/STAFF FORENAME, 19/STAFF ADDRESS, 20/STAFF HOURS PER WEEK, 21/STAFF PAY, 22/MEMBER TOTAL COST. 23/MEMBER BANK ACCOUNT, 24/MEMBERSHIP ENROLLMENT DATE.
When a new member joins his or her fitness is assessed and data placed in feilds 10 - 13.
1 member may enrol on many courses we offer. 1 staff may teach many courses and many members.
Feild 22 is SUM total of all courses member has enrolled upon. Feild 23 is 14 digit members bank account number.
All costs must be in pounds sterling.
Use UK format for dates ie: dd/mm/yyyy
Deliverables:
Two versions of the database.
Version 1: the database normalised to 5NF. Must run on MS ACCESS XP.
Version 2: the same database which can be operated from a VBA front end. User can use the front end to access data about members. Add new records, delete selected records. And print relevant details.
Front end must have the ability to display all memberships details including staff details related to that member. Also must give user the option to query DB to search for the following, display members beginning with a certain letter of the alphabet which the user will input. Display members over a given height AND weight. Display all members on a given course. Display members enrolled AFTER a given date.
Furthermore provide me with a word documnet detailing the normalisation process form un normalised throught to 5 NF including all feilds in between. Stiplulating all assumptions and logic made for each normalised form.

Deadline legal notes: All times are expressed in the time zone of the site EDT (UT - 5). If the buyer omitted a time, then the deadline is 11:59:59 PM EDT on the indicated date.

I have gone through your spec and feel confident of delivering this project with Best results as well.
I can covert the fields from unnormalized through to NF5 and document the process in a word document. And also, develop the relevant forms in MS-Access.
