I've made a draught of what you asked for, I hope it meets your needs. The attatched zip contains the file in MS Word format. Unfortunately RentACoder seems to have a problem in that it wont let me upload the full file (including screenshots) so I'm sending a version without the graphics. If you accept my bid (or if rentacoder get the site fixed) I can send you the full version via e-mail. Of course, feel free to contact me any time (replying privately to this bid request will effectively send me an e-mail) about any revisions you need, or anything else.
