Description:
MS Access database with the following fields
· ID Number
· Date Added
· Last Name
· First Name and Middle Initial
· Address (2 lines)
· City
· State
· ZIP Code
· Telephone Number 1
· Telephone Number 2
· Fax Number
· E-mail Address
b. Index the database on the ID Number and Last Name + First Name, so that the database can be searched by ID or Name. (2pts)
c. Write a Visual Basic program that can add, update, and delete records in the Microsoft Access database. (5pts)
d. Enter 10 different records. (2pts)
Deliverables:
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.
3) Complete ownership and distribution copyrights to all work purchased.
4) Completely document code for VB program.

So if this is what you are wanting i have it done, and can give you all 4 parts for the listed bid price on this bid. If you just need just this part I can also sell you that. Just get back to me as soon as possible and i'll get you the source code. Its all tested and completed.
