Description:
Attached is a simple order form sample. Can you quickly revamp this page with a nice, professional layout? This small project may well lead into a larger web form design project.
Feel free to do anything you want with the design as long as it runs on IE 5.5 and higher (and, preferably, Netscape 7). This is an isolated page so it doesn't need to comply with any overall theme--something abstract is fine. If you need a more concrete theme, this page will relate to products involving "Television and Movies".
Note that (javascript) form validation is not needed for this (though you can use javascript for other things if you need); I'm just interested in a good, user-friendly layout based on this sample form.
Please send a link showing a sample of previous page design work if you have any. Looking for a professional web page designer that can do this quickly. Thanks for your time.
Deliverables:
1) All html code and supporting files. Any graphics should be delivered in jpg, gif, or png format.
2) Complete ownership and distribution copyrights to all work purchased including any graphics artwork.
