I am starting a beta testing period for a new software product. I need a form written in ASP that will allow beta testers to enter the results of there testing. When the information has been entered on the page, the user can click OK and the data will be sent to a Microsoft Excel file. I will be able to download and examine the Excel file. The web site has been built in FrontPage and is hosted on a service which has the FrontPage extensions installed. However it is not necessary to use them as long as I can format the looks of the page in FrontPage. I will provide the details of how the form should look and what it needs to capture. There will be drop down lists with fixed choices to make and fields where text is entered. I will make available the server site so you can test your results. You&'ll need to sign an NDA when you are assigned the project. Deliverables: 1) Complete and fully-functional working scripts and complete working web pages, along with sample Excel file containing results. 2) Complete ownership and distribution copyrights to all work purchased.
