Description:
I try to generate a PDF-File "by hand"...
This works well so far.
But I have problem with XREF-Section in combination with Graphics.
Without XREF file can be opened at Reader. But with XREF there is an error....
Need somebody who know the file internals of PDF to help me.

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

Both files with graphics have the length of content stream incorrect. It is 5482, it should be 5522. Acrobat is more able to fix 'damaged' files without xreference tables, by some reason. Here is the corrected (by me, in hex editor) version of your file (attached). The changed length value is at file offset 92320 (decimal).
