Normally this is administered online as ASP pages. However, we want an emergency backup version to be hosted on a different host in case the main host fails. We cannot have the entire database up live on the backup host, so we need a more low-tech version that can stand on its own. We need someone to make a forms handler for the existing PDF form or HTML version of the assessment that will save the answers in a Flat Text format, so that they can then be downloaded from the Web server and imported into SQL or Access. All questions in the assessment are required to be answered by the candidate. Although multiple choice, two sections of the assessment (77 questions total) require TWO answers per question: a "Most" and a "Least" answer. Other than that, all questions require only one answer each. The only other fields on the form are several text boxes at the beginning for the candidate's name and other demographic info. Deliverables: 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased. I would be delivering the ASP Pages in 4(four) days flat. Here is what I propose for you. 1. Either I would modify the action property of the forms in the PDF Files to point to the new asp pages or I would make all the input pages from the PDF Files. 2. Develop the ASP pages to accept the input from the pages mentioned in 1 and store it either in MS Access or Text File or XML. The format of the text/xml file would be finalized after discussing it with you. 3. The coding would be as per industry standards and would be heavily commented. Kindly clarify the following 1.  I'm assuming that these are free quizzes i.e. user does not have to log in (the reason why i ask this question is to find out whether I need to pick user validation information from some other data source) Please note that this job is technically simple but is slightly effort consuming due to the number of questions (fields in the db) that need to be properly handled.
