Description:
I want to make a simple web game where people are asked questions, and they answer with a 'checkbox' multiple choice, then the next screen shows a picture, a message, and another question(s). User can also add his own answer in a "form field". Images should 'fade' out when user selects answer and next screen comes up. (javascirpt?) Game ends at certain stage and user can restart game or go back, view his 'score' against others, and view answers that other users have put in. Thank you. I have PHP and PERL on my server, so you can use either one.

Deliverables: 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.
3) Complete ownership and distribution copyrights to all work purchased.
