Description:
CRITICAL FACTOR: Time. Need it YESTERDAY.
A simple HTML form page collects the following info (page will be made pretty later) that is processed by a PHP script:

Remote Name: 208.3.65.173

Date: 09.06.2002
HeardFrom: 150 characters line of text for telling us where they came from/how they found us
notifyemail: someemail address
It MUST use these feild names. It should verify email address (remote name is the IP address of the user)
script sends the above info to TWO email address:
--- one email address is hard coded in the script (the notifyemail)
--- the other is a hidden field on the form page.
It also ADDS this info to a CSV file in cgi-bin (fields are in the same order as above, comma delimeted).
yes, I just need to confirm the emails are in the correct format.
I have one requirement I forgot:
a second hidden field 'ID'
will contain a number. I need the users email address (not the notify email address, the user supplied email) appended to a text file named after that ID:
i.e., add 'emailaddress,' (WITH COMMA!)
on seperate line in '12345.txt'
Deliverables:
Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
Complete copyrights to all work purchased.
Platform:
web script, UNIX/PHP. If it worked on NT AND uNIX that would be VERY nice.
