Description:
I need a script that will allow users to send me the form that they have filled out on my site. I am on a NT server running php. I would like to have a form validator too . the form does not contain any sensitive information. I would like a program that uses the varables that I define.
Deliverables:
Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
Complete copyrights to all work purchased.
Platform:
This program needs o run on a server running windows nt 5

Deadline legal notes: All times are expressed in the time zone of the site EDT (UT - 5). If the buyer omitted a time, then the deadline is 11:59:59 PM EDT on the indicated date.
Special Conditions / Other:
There is a rush for this project but the time frame can be expanded once work has begun,but no more than 2 more weeks
Remember that contacting the other party outside of the site (by email, phone, etc.) on all business projects < $500 (before the buyer's money is escrowed) is a violation of both the software buyer and seller agreements.
We monitor all site activity for such violations and can instantly expel transgressers on the spot, so we thank you in advance for your cooperation.
