Description:
I want a program in VB that will ask the user questions (not more than 20 questions which I will provide in MS Word fromat), and then based on the answer (I will recieve one yes or no answer), either tell the staff member to notify me by phone and then fill out the form, or fill out the form directly.
The form is currently in MS Word 97 format, but I would prefer that it be enhanced to have a small database so drop-down boxes (comb-boxes) can be used to fill in the form. There are approximately 40 fields on the form including:
12 drop-down fields, which should allow an option to accept non-listed data (add data)
6 date/time fields
5 text fields (no drop-down needed)
and various other fields that are mostly check boxes or options boxes
They will need to be able to add and delete data because the names/data will change over time, but will not exceed 50 names total for each field. After the form is filled out, I want one version printed to thier printer with instructions to fax it to me, and another version with the patients name stripped out (for patient confidentiality reasons)and emailed to me using whatever email program they use. The users all seem to have Word 97 installed, but may not neccessarily have email, or may not have it online, and the program needs to remind them to email it if they can, but be prepared to give up if they don't.
I would like to be able to change my email address remotely, but that is not a neccessity. I can program somewhat, but I not do it for a living so I am slow, so I am looking for something either well commented and/or with very meaningful variable names.
If we can work this out to our mutual satisfaction, the next step would be to complete an access database that I already have partially completed, and a conversion program to add the emailed data to the database.
I am not running a business,I work for a non-profit company and this would help me in my work.

Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
Complete copyrights to all work purchased.

Windows (95 and up including NT)
NOT web based. (I don't have a server)

Deadline legal notes: All times are expressed in the time zone of the site EDT (UT - 5). If the buyer omitted a time, then the deadline is 11:59:59 PM EDT on the indicated date.
Special Conditions / Other:
Must be completed by 10/1/2002
in Visual Basic. must be SIMPLE with no complicated dependencies, DLL's tricky coding. may use MS Word for the form, but must be able to be changed when needed to use other versions of Word.
