Description:
I am developing a site which will offer rebate coupons to a customer. They will input user data to a form or table which will send the data to an MS Access data base. The input data will then be automatically entered into a form which is on the rebate coupon. This completed coupon will then be emailed back to the customer. Also emailed, simultaneously with the coupon would be two additonal documents that would be filled in with some of the data collected via the request form. An email advising of the request submittal and automatic response would be emailed to the sales department of my company for followup.
Please respond asap as this project is time sensitive.
Deliverables:
Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
Complete copyrights to all work purchased.

FrontPage 2002 on machine running win98SE
Browser is IE5.5

Deadline legal notes: All times are expressed in the time zone of the site EDT (UT - 5). If the buyer omitted a time, then the deadline is 11:59:59 PM EDT on the indicated date.
Remember that contacting the other party outside of the site (by email, phone, etc.) on all business projects < $500 (before the buyer's money is escrowed) is a violation of both the software buyer and seller agreements.
