Description:
I need a simple form made in Visual Basic 6. I need it to include areas for First Name, Last Name, Address, City, State, Zip code, and Phone Number. Once it's finished being filled out i need it emailed to an email address in this format, First Name: Brian (belowe it) last Name: Lindquist (Belowe last name) Address:ffsdfsd . I need the info entered to be sent to general AT fifmo.com sorry but i cant use the at sign
Deliverables:
Complete and fully-functional working program(s) in executable form as well as complete source code of all work done in Visual Basic (Preferd but not required in Visual Basic). Complete copyrights to all work purchased.
