Description:
Data edit/entry form with two macro buttons. One button to preview the report and another to print two one page reports.
Deliverables: 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.
3) Complete ownership and distribution copyrights to all work purchased.

Deadline legal notes: All times are expressed in the time zone of the site EDT (UT - 5). If the buyer omitted a time, then the deadline is 11:59:59 PM EDT on the indicated date.
Remember that contacting the other party outside of the site (by email, phone, etc.) on all business projects < $500 (before the buyer's money is escrowed) is a violation of both the software buyer and seller agreements.

Hello,
Do you want a macro attached to buttons or VB code?
VB Code is much better because it would be much more flexible in the future.
What you need is extremely simple in either way, but I would need the names of the reports and the confirmation that the field names of the Patients table are the same as the labels you have in the screenshot (ie field that holds Patient's name is [Patient Name].
In your screenshot there is no Primary Key (field that uniquely identifies a patient). Is there any? If there is not, you may end with displaying two patients with the same name, which happens very frequently.
