Text file generated in report format, would like to extract 'pieces' of data from the report and write to a table in Access.
Example of 1 page from report with header on each page (sorry but the text is wrapping when it is posted here):

I need to pick up the phone number, name and credit rating fields while ignoring headers and other lines of text. They are always same number of spaces from left but have 4-5 lines of data between each record. Report may be 5 pages or 100.
Header consists of 4 lines, footer to each record are the dashes, record itself is the 4-5 lines.
Expected data extraction results from above example data should be in comma-delimited format like this:
EVELYN T BARTERS,423-634-1154,U
TIMOTHY C BUMGARD,423-825-0215,U
Can provide example file if needed for quote.
Deliverables:
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done as described.
2) Complete ownership and distribution copyrights to all work purchased.

Must be 100% finished and received by buyer on:

Deadline legal notes: All times are expressed in the time zone of the site EDT (UT - 5). If the buyer omitted a time, then the deadline is 11:59:59 PM EDT on the indicated date.
