Description:
I would like to find someone to help on this first project and possibly do more projects on a regular basis afterwards if he/she performs well.
Today I need a VB module to provide a way to present a user with questions in sequence and then either a multiple-choice combobox or free-form textbox to answer them with. This runs on SQL Server(MS) I will supply the skeleton project ready to go with the SQL routines/interface and my naming conventions to help guide you. Prefer if you have access to your own MS SQL Server, but I can also allow you to test on one of my empty servers if you don't have one. Attached is the basic outline in Word plus the sample database(exported in Access).
Deliverables: 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.
3) Complete ownership and distribution copyrights to all work purchased.

I have looked at your request and it is well within my experience.
Based on your specifications, I would supply the following:
- A VB popup dialog form to process the question and answer session
- If this is the entire project (and not part of a larger project), then a vb project, executable and installation package will be provided.
(Please specify level of Windows operating system that this will run on.)
- A VB class module that encapsulates the data handling and exposes a question and answer session object. The form would use this class.
This way you can easily transport the logic to another user interface at a later stage (i.e. web page, etc)
- Example code for opening and using the dialog from within a Visual Basic application
- bonus: I would also code for a third component type 'Option/Combo' that would automatically use option buttons instead of a combo box for lists with items less than a specified number (e.g. 5 items).
I believe this would ease user input
- All code will be well-commented and any required installation notes supplied
I have just logged into this site so have not done a resume as yet.
In brief:
I have 14 years commerical experience.
5 years with VB5-VB6
10 years with MS Access and MS Sql experience within those 10 years.
6 years as a freelance developer, small to medium windows desktop systems
I am available for immediate start and would have the app finished within the day
I have MSSQL 7.0, VB6 and Access 2000.
