Description:
I need written an action page for the attached form, which is HTML in a text file. The data for that form needs to be sent preferably using CDONTS or some other mail component (IIS is the server).
Since this should be a cake job, I'm offering $10. Just write a quick action page that collects the form data and sends it to an arbitrary email address. Then of course paste the form I've written into a page and direct it to the action page. I'll test it on my server after you write it.

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.
