Description:
I need a web based program created that allows us to track customer refunds posted by several virtual staff members.
This program will be located inside a password protected area on our server. Since no credit card information will be entered into the program it does not need to be on a secure server.
Ideally the program could post the data to a .xls file in the background so after we enter the data we have the option to "download the .xls or .csv" file to our hard drive to save a copy.
This program would be accessed a couple times a week via a web browser.
I want to be able to click a button that says "SEE DATA" and see all entries.
As new customer refunds are put through our merchant account by our staff they would then login to this program online and manually enter the new refund details into the system by 1 staff member.
The data form fields in the program would include:
Customer First/Last Name
Customer Email Address
Registration Code
Password
Refunded Amount
Reason for Refund
Exceeded Refund Limit (yes or no checkbox?)
ALL fields but the name and email will be optional in case I'm in a hurry and don't have time to fill in all the info right then and there.
The "reason for refund" form field will allow up to 200 characters or less, such as "bought wrong product" or "duplicate order" and so forth.
I would like the program to automatically insert that days date along with each entry, if that isn't too time consuming to add in. (optional)
We had a similar program written awhile back that posted the data to a spreadsheet so I think it's possible to do this for this project too. In the very least we need to be able to easily "download" the data in some format.
I would really like the data posted to a downloadable spreadsheet simply so we can "search" on entries.
The end user interface doesn't need to be anything special. It can be simple.
Open to suggestions on this!
Deliverables:
1) Create a fully-functional working web based program that meets the above requirements and contains the above "required" elements and features.
2) Install this to our server and test it to ensure it's working 100% correctly.
3) Write up installation instructions (step by step) in case we ever need to reinstall this or have it installed on multiple domains.
4) Zip up the original source code/installation files and send those to us when the project is complete.
5) All rights to the original source code, finished program and anything related to this project will be the sole property of our corporation.
6) You agree that we retain all rights to the files and can resell the program if we so desire in the future.
Platform:
It needs to be web based which can be accessed via a password protected directory on our server via a common web browser such as IE and Netscape 3.0+
The data needs to be posted in the "background" to a .xls file if possible, or be "downloadable" as a tab delimited or comma delimited .txt file which we can them import into Microsoft Excel.
Must be 100% finished and received by buyer on:

Deadline legal notes: All times are expressed in the time zone of the site EDT (UT - 5). If the buyer omitted a time, then the deadline is 11:59:59 PM EDT on the indicated date.
Special Conditions / Other:
The deadline to have this project completed is December 30th/2002 however earlier is always better.
I'd like this to be started right away and in case we need to discuss the project via the chat room here I need to be able to do that during regular US business hours so someone in Canada or the US is preferred.
I am open to suggestions on this project such as additional features you think would be good to include or things that you think I've asked for that will cause unwanted delays or complications during the creation of the program.
Remember that contacting the other party outside of the site (by email, phone, etc.) on all business projects < $500 (before the buyer's money is escrowed) is a violation of both the software buyer and seller agreements.
We monitor all site activity for such violations and can instantly expel transgressers on the spot, so we thank you in advance for your cooperation.

I've created a demo for you to take a look at based on the requirements above:

I've added a search function into the program as it should prove usefull, and save downloading into excel most of the time. You can of course still download the data in CSV format as you'll see in the demo.
I can get this completed for you well before the deadline, the time only depends on what additional functionality you require (the option to delete entries for example).
I tend to work during US business hours anyway so the timezone difference shouldn't pose any problems.
