Description:
I would like a pre-packaged or custom designed form designer that will allow editing forms for changes and saving the results to several different Access databases, depending on which items are selected.
Added for clarification: For example, if a customer selects one group, the results would go into database1.mdb, if he selects a second group of choices, the results would go into database2.mdb, etc. Each database would be independent of each other so that they can be sent to the appropriate parties after the form deadline. If the customer selects one item in Amateur Sno Cross, it would put the entry into the database1. If he selects one item in Sno Cross and one item in Ovals, then it puts an entry into each database, etc. There would be a total of six (6) different categories for this form.
Results to also be emailed to specified address & recipient would receive two emails if customer selected an item in group1 and another in group2, etc.
I may have to start a generic HTML form to give an idea of what I am looking for. Thanks for your bids!
Deliverables: 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.

Special Conditions / Other:
Must be compatible with all major web browsers, i.e. Internet Explorer 5.x and Netscape 4.7x and newer.
Remember that contacting the other party outside of the site (by email, phone, etc.) on all business projects < $500 (before the buyer's money is escrowed) is a violation of both the software buyer and seller agreements.
We monitor all site activity for such violations and can instantly expel transgressers on the spot, so we thank you in advance for your cooperation.

Hey i'm interested in your project. So you'll have a form that has up to 6 groups of drop downlists and each drop down list will populate the database for that categeory that was selected and e-mail to a message based on that category?
I'm an Professional ASP programmer. I have done several ASP applications, websites, and code snipplets to be use as addins. You can have look over my ratings to check all that out if you would like to. I always just good standard programming style and methods. By using this style it makes the code easier to read, understand, and debug if any errors occur. You will be able to tell what kind of data is stored in each variable and how its being used.
- I always use Option Explicit.
- Declare all variables.
- Use Proper Naming Conventions.
- Use Code Indentation (3 spaces).
- Close all Connections/Recordsets.
- Add error checking
