Description:
I am seeking an "existing" web based Appointment application along the lines of the images I have created and enclosed. Whatever you have should be able to be modified to look like this - or close.

1) It is an application for individuals to use. These people would come from my database so there must be a simple way of plugging this into my DB.

2) Each person will manage their own Appointments so the appt Table in the DB must accommodate multiple clients.

3) It must also record who the Appointment is with. This information also comes from my DB which contains a list of available names.

Deliverables:
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request.

3) Complete ownership and distribution copyrights to all work purchased.
