Description:
I need a employee review application that will transfer into a word document and an access mdb.
The first form, frmEnter, would ask if it's an annual or 6-mo review. Then, if it's a manager or staff review.
For a manager review there will be 6 categories (leadership, etc) that the user will type in their comments and then rate the person from 1 to 15.
A staff review will have 5 categories (productivity, etc) that the user will type in their comments and rate the person from 1 to 15.
When they have typed in all the details I need it the application to produce a word document that has the category, rating, and comments in it.
Lastly, after the review is done i need a SUBMIT button to load the review into an access database.
I need this to work for windows 95 up to windows 2000. I would prefer vb6 using word 2002.
Deliverables:
Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
Complete copyrights to all work purchased.
